Basic Education Enrollment Info

Requirements:

1. Original Report Card (Form 138/SF9)/Official Transcrip of Records & Honorable Dismissal
2. Original Certificate of Good Moral Character
3. Photocopy of PSA Birth Certificate
4. Photocopy of Baptismal Certificate

Additional Requirements:

• For Grade 11 Student Applicants from Private Schools: Voucher Certificate
• For Grade 7 Night High School Applicants: Certificate of Source and Annual Family Income

ENROLLMENT PROCESS:

STEP 1: Enroll Online here and attach the required documents.

STEP 2: Upon approval, you will receive the Notice for Enrollment QR Code in your profile and email.

STEP 3: Pay online or in partner stores the PHP 2,000 enrollment fee.

Upon receipt of payment, you will receive the following:

• Student Number and Password
• You will also receive your USI Gmail Account and Online ID.

STEP 4: Accomplish the Registrar's Form.

STEP 5: Bring the required documents to the Admission Office for final validation.
